Incident Report Request

CFD2 will provide a copy of an incident report upon request. Effective Jan 1, 2018, the following fees will be charged for such reports. Reports will be issued once payment is processed. Checks should be made out to Consolidated Fire District 2 and submitted to our Administrative Offices at 3921 W. 63rd St, Prairie Village KS 66208. Credit card payments are also accepted at our Administrative Offices or over the phone.

If requesting photos along with a written report, add the appropriate fee as listed below depending on which format you choose.

Photos: Sent via Google Drive – $10, flash drive sent via USPS – $20
